Advocare Marlton Pediatrics uses screening tools at many of your child’s checkup appointments. The forms ask questions about your child’s development and whether they are able to complete certain tasks. For your convenience, we have made these assessments available on line. Printing them in advance allows time to accurately answer each question, while also saving you time in the office. Please select the forms appropriate for your child’s age and give your completed forms to the front desk upon arrival for your appointment. Activity Sheets are also included for each age range. You may find these helpful to work with your child, but they are not required for your visit.
